What BBL actually is, in our hands.

BBL stands for Brazilian Butt Lift — a name that has become culturally larger than the operation itself. The procedure is, mechanically, fat transfer: liposuction harvests fat from areas where you have it (typically abdomen, waist, and thighs), the fat is processed, and a portion of it is re-injected into the buttocks to add volume and reshape the contour. The waist looks smaller, the buttocks look fuller, and — if the procedure has been done well — the proportions read as natural rather than placed. The harvest and the injection are equally important; the result is as much about what we remove as what we add.

BBL has had a difficult safety history globally, particularly when fat is injected too deep and reaches the gluteal vein system. We perform the procedure under strict depth control: fat is placed exclusively in the subcutaneous layer, never into or below the muscle. This is the only safe way to do the operation, and it is the only way we do it. We use cannulas with depth markers, ultrasound guidance where appropriate, and we operate at deliberate, conservative volumes — not the maximum the patient can hold.

Not every patient is a BBL candidate. The honest part of this consultation is telling you when your anatomy — or your lifestyle — means a different answer is the right one. Aesmax

The other distinctive thing about BBL is the recovery: you cannot sit directly on your buttocks for two to three weeks after the operation, while the transferred fat establishes a blood supply. This single constraint shapes the recovery more than pain or swelling does, and it is the most important practical question to plan around — particularly for patients flying in for the procedure. We will tell you, in consultation, exactly what the no-sitting period will require of you. If your work or life cannot accommodate it, this is the wrong procedure for you, and we will say so.

From the day of surgery to the final result.

Day one to two

You wake in your hospital room with a compression garment in place over the harvest areas (abdomen, waist, thighs) and a strict no-sitting-on-buttocks rule already in effect. Pain is moderate — mostly from the liposuction sites — and well-controlled with oral medication. You sleep on your front or side. Walking short distances is encouraged. We see you the morning after surgery before discharge.

The no-sitting period

For the first two to three weeks you cannot sit directly on your buttocks. You sleep prone or on your side; you eat standing or kneeling at the table; you drive (when permitted from week one) using a specialized BBL cushion that transfers weight to the back of the thighs. This is the part of the recovery that makes BBL different from every other procedure on this site. It is non-negotiable: sitting too soon kills transferred fat and ruins the result.

Sitting returns, gradually

From week three onward, sitting on the buttocks is permitted in short increments — ten minutes, then twenty, then longer — building back to normal sitting by week six. The compression garment over the harvest areas continues for six to eight weeks. Most patients return to office work in this window; sitting at a desk is the rate-limiting activity, and the BBL cushion stays in use until full sitting tolerance is established.

The fat takes

Of the fat transferred, a portion (typically sixty to seventy percent) survives long-term; the rest is reabsorbed by the body over the first three months. This is expected and built into the volume we transfer at the operation — we do not transfer a final-result volume; we transfer the volume that, after partial reabsorption, becomes the final result. The buttocks look largest in the first weeks and then settle.

Exercise returns

Light cardio (walking, stationary cycling) from week four. Lower-body resistance work waits until week eight to twelve, depending on how the harvest areas are healing. Patients who return to heavy exercise too early can disrupt the harvest sites and the contour gain in the waist; we assess readiness directly at your check-ins.

The final shape

Between months three and six the surviving fat has stabilized, the harvest areas have fully contracted, and the proportional change — smaller waist, fuller buttocks — reads as it will read long-term. We see you at one week, three weeks, six weeks, three months, six months, and twelve months. Photographs at the twelve-month visit are how we judge a result, not how we judge it at three weeks.

Q.01

Is BBL safe in your hands?

Performed correctly, yes. The serious complications associated with BBL globally come from fat being placed too deep — into or below the gluteal muscle — where it can enter the venous system. We place fat exclusively in the subcutaneous layer, never deeper, using cannulas with depth markers and conservative volumes. We operate at one of the highest patient-safety standards available in Istanbul, in a JCI-accredited hospital, with an anaesthesiologist on-site throughout. The technique decisions that make BBL dangerous in some hands are decisions we do not make.

Am I a candidate for BBL?

Not everyone is, and we will tell you directly. You need adequate fat in the harvest areas (abdomen, waist, thighs) for there to be enough to transfer. Very lean patients are not BBL candidates — there is nothing to harvest. Patients whose lifestyle cannot accommodate two to three weeks of no-direct-sitting are also not candidates, regardless of anatomy. We assess both in consultation. If the answer is no, we say so.

How does the no-sitting period actually work?

For two to three weeks, no direct weight on the buttocks. You sleep on your front or side. You eat standing, kneeling, or leaning. When you must sit (for example, in a car), you use a specialized BBL cushion that transfers weight to the back of the thighs and leaves the buttocks unsupported. From week three you can sit in short increments, building back to normal by week six. We provide written guidance and the cushion before you fly home.

How much of the transferred fat survives?

Typically sixty to seventy percent long-term. The remainder is reabsorbed by the body over the first three months. This is expected and accounted for in surgical planning — we transfer the volume that, after partial reabsorption, becomes the final shape. The buttocks look largest in the first weeks and then settle to their long-term volume.

Will the result last?

The fat that survives the first three months is your fat — it stays. It will respond to weight gain and weight loss the way the rest of your body fat does; significant changes in body weight will change the volume. Most patients keep the BBL result indefinitely; some return for a small top-up several years out if they have lost weight in the interim.

How soon can I fly home if I am coming from abroad?

We ask international patients to plan ten to fourteen days in Istanbul. The flight home is the harder part, because the no-sitting rule still applies on the plane — you will need to use the BBL cushion throughout the flight, and to stand or recline as much as possible. We see you the morning after surgery, again at three to five days, and once more before you fly. Long-haul flights are safe from day ten.

Yes — the gallery on this site shows cases under explicit patient consent, and we can share additional comparable cases on request, privately. We do not publish photographs without patient permission, and we will not share other patients’ photographs by WhatsApp without first asking them.
